This information is collected annually or as necessary. Please complete and submit the form below so that we can ensure our records are accurate. All questions asked are required under our accreditation with the Standards for Excellence® program by the Standards for Excellence Institute.

Please note that the Delaplaine keeps private information confidential per policy. However, the names of Board members are considered public information (and are required on publicly available legal documents, such as the 990). Further, addresses and contact information of Board members are required for some grant applications (including county, state, and federal grants). This information is not made available to the public. Other information provided may be used in aggregate for grantmaking, for the Delaplaine’s GuideStar profile, or for other surveys that may benefit the Delaplaine.
